Hospital at Home (Previously known as Virtual Ward)

ELM have been involved with  the Hospital at Home Service since it was launched in March 2023. Working in  partnership with South Tees Foundation Trust  we provide GP cover to the service to  enable patients to receive an  enhanced level of care in their own homes, with the aim to allow earlier discharge from hospital and where appropriate to avoid admission in the first place.
