Your information – how we use it

During episodes of clinical care we will record information about your healthcare examinations undertaken and treatment or advice offered onto our clinical system. With your permission we will also be able to access your medical record from your own GP to ensure we are offering you the safest and most appropriate clinical care we can.

We may use you data for audit purposes in order to improve our services, your information will stay within the organisation wherever these audits are conducted

How we keep your records confidential

Your clinical information is available only to our staff who have all been trained in their duties of confidentiality. Access to clinical information is controlled by role-based smartcard permissions.

Everyone working for the ELM Alliance must comply with the Common Law Duty of Confidentiality and act in accordance with the Data Protection Act. Information you provide to us in confidence will only be used for the purposes explained to you and to which you consented, unless there are other circumstances covered by the law. All our staff are required to protect your information, inform you of how your information will be used, and allow you to decide if and how your information can be shared. All our staff are trained in information governance.

Sharing information with other organisations

We will share your personal information with other organisations for healthcare purposes only. This may be with your general practitioner, district nursing, health visitors and school nurses and other relevant primary care agencies as well as with local hospitals and local ambulance services for the purpose of your direct medical care.  We will inform you what we are going to share and with whom.

Sometimes the law requires us to pass on information e.g. when we encounter infectious diseases that may endanger the safety of others or when a court order is received.

By law you have the right to request access to your health records pertaining to your treatment from our service.  You can do this by contacting our Head of Governance on 01642 511333.
